COMPARATIVE ANALYSIS OF THE DIFFERENT CRITERIA FOR POST-LAPAROSCOPIC RADICAL PROSTATECTOMY SEXUAL FUNCTION EVALUATION: REVERSE SYSTEMATIC REVIEW.
Resumo
Objective: Primary goal was to identify the most common definition criterion for post-prostatectomy erectile dysfunction (PPED) after laparoscopic radical prostatectomy (LRP) and secondly, to compare the rates of PPED over time under different criteria.
Methods: In the period from January 1, 2000, until December 31, 2017, we used a new methodology to perform our evidence acquisition called reverse systematic review (RSR). The PPED rates were evaluated at 1, 3, 6, 12, and 18 months after surgery in the different criteria selected as the most commonly used, grouped in three: Erection sufficient for intercourse (ESI), IIEF-5, and Others. Moreover, the RSR showed the “natural history” of PPED after LRP.
Results: The systematized search found 353 review articles. After filtering for duplications and inclusion/exclusion criteria, 40 reviews were chosen that cited 62 primary articles on sexual function after LRP. A total of 83 reports were selected and comprised a total number of 21.618 patients. The difference was statistically significant at the 6th month with a superior erectile function rate detected by IIEF-5 of 64.8% and by other methods at the 18th post-operatory month analysis of 70.6%. No statistical difference in erectile function rate between different criteria was found when assessing early potency recovery.
Conclusion: RSR showed the natural history of PPED criteria selection through time, and how they could be compared. Demonstrated also the similarity of results between more complex scores and simple one-question criteria.
